Step-by-Step Application Process
- Create an account on gov.pl and obtain Profil Zaufany
- Log in to MOS (Modul Obslugi Sprawy) portal
- Fill in the application form and upload documents
- Pay the stamp duty (440 PLN for the card)
- Receive confirmation email with your case number
- Wait for the Voivode decision (up to 60 days for urgent, 90 days standard)
- Collect your physical card at the Voivode office
