It's 9 p.m. in a Warsaw hostel kitchen. Ravi has his appointment next week, his photos, his employer letter. Then a roommate says: “No EUR 30,000 insurance, no karta pobytu.” Someone else shrugs: “ZUS is enough, don't waste money.” Ravi opens his phone and types the question you're probably typing right now: what is the real rule on health insurance for karta pobytu 2026? Short answer: both roommates are half right. Which half applies to you depends on one thing — what your contract says, and whether anyone has actually registered you. This post separates the two routes, shows where the number 30,000 comes from, and gives you a checklist for application day.
So, what does the office actually want to see?
Start with the rule itself, not the rumours. For a temporary residence and work permit (Karta Pobytu, the Polish residence permit), you need health insurance within the meaning of the Polish act on publicly funded health care of 27 August 2004 — or a confirmation from an insurer that it will cover your treatment costs in Poland. That is the wording on the voivodeship information cards, and the general framework sits on gov.pl/web/cudzoziemcy.
Here is the part that saves most people money. If your application includes a statement that you will work under an employment contract or a contract of mandate (umowa zlecenie) that guarantees health insurance, you do not have to attach separate insurance documents. The office treats the requirement as met through the work itself.

That is the law on paper. In real life the contract alone is not magic. Insurance has to be real — you are registered, and the contributions are paid. We'll get to that in a moment.
Where does the EUR 30,000 number come from?
Not from the work-permit rules. The EUR 30,000 figure comes from the Polish rules on travel medical insurance for visas, and Polish embassy notices repeat it for people applying for long-stay documents. That is why it travels around hostels and WhatsApp groups as a universal law. It isn't — but it is a very useful benchmark when you need a private policy.
According to Polish foreign-ministry notices on gov.pl, a policy that meets the benchmark has to do all of the following:
- Insurance sum of at least EUR 30,000.
- Covers the cost of return to your home country for medical reasons, emergency medical assistance, sudden hospitalisation, and death.
- Valid for the whole planned stay, not only the first months.
- The insurer agrees to pay the health-care provider directly, on the provider's invoice.
- A 24-hour alarm centre that accepts reports of insured events.
So when does this matter to you? When you have no ZUS-registered work behind your application, or when you are covering a gap. In that case, EUR 30,000 is the number people aim for, and the safest move is to check your own voivodeship's checklist before you pay anyone. A cheap tourist policy bought “just in case” that doesn't pay the clinic directly is a classic waste of money.
ZUS route: how do I know I'm really insured?
Being on a contract and being insured are two different facts. Most problems start in the gap between them.
Your employer has to register you with ZUS — for a umowa zlecenie, within seven days from the day the insurance obligation starts, on the ZUS ZUA form. Health contribution is 9% of the contribution base. The official position, repeated on the government's green-line portal, is that your obligation counts as fulfilled only after you are registered and the contribution is paid on time. With registration and payment in place, you get the same public care as a Polish citizen through NFZ.
Don't wait for the urząd to discover a problem. Ask your employer for three things in the first month:
- A confirmation that you were registered for health insurance, with the date.
- A payslip or settlement showing the health contribution was deducted and sent.
- The name of the person in HR who handles ZUS, in case something changes.
One more trap: the insurance question and the pay question are separate checks. A perfectly registered contract can still fail on income. If you're on a mandate contract, read Umowa Zlecenie vs Umowa o Pracy for Karta Pobytu 2026 and Salary Threshold for Karta Pobytu 2026: PLN 4,806 before you assume the paperwork is fine.
An illustrative case, generalised from situations we see: Anita, a caregiver from the Philippines, signed a zlecenie in March. Nobody sent the ZUS registration. She only found out when she tried to book a GP and the system showed no insurance. Her employer registered her the same week, she collected written confirmation, and she walked into the urząd with a document instead of a promise. It was a small fix — but only because she checked before the appointment, not after.
What about my wife and kids?
Employees and their family members have the right to use public care on the same terms — but family members have to be reported by the employer. Your spouse isn't insured automatically because you are. If you're bringing them, plan the insurance at the same time as the permit, and read Family Reunification in Poland 2026 for the timing.
When does a private policy or voluntary NFZ make more sense?
If ZUS covers you from day one, you rarely need to pay twice. The exceptions are the moments when your ZUS cover is missing, delayed, or about to end:
- The first weeks of a new job, before the employer has sent your registration.
- A gap between two jobs — for example, you were let go and are looking for the next employer.
- A family member who hasn't been reported yet, or who arrives before you start work.
- A contract type that doesn't create an insurance obligation at all.
The gap-between-jobs case is the scariest, so read Lost Your Job in Poland? 15 Working Days to Tell the Urząd and think about insurance on day one of the gap, not day thirty.
Voluntary NFZ insurance is the alternative to a private policy. Its price moves, because the base is the average wage in the enterprise sector and it is updated every quarter. For April–June 2026 the minimum monthly premium was 835.04 PLN, and for July–September 2026 it is 845.67 PLN. We can't responsibly quote the October–December figure, so check the current quarter on nfz.gov.pl. The premium for a month is due by the 20th of the following month.
How to choose? A private policy usually costs less but pays only within its limits and its exclusions. Voluntary NFZ costs more but gives you public access like any insured person. Ask every insurer for a written quote and a policy wording in English, and check the five points from the benchmark list above.
Practical tip: Put one folder on your phone called “Insurance” — registration confirmation, last payslip showing the health contribution, and the policy PDF if you have one. When the urząd asks, you answer in thirty seconds.
What do I carry on application day?
Every voivodeship has its own checklist, so confirm yours. But this short list covers the insurance side for almost everyone:
- Your signed contract (employment contract or umowa zlecenie) and the statement that it guarantees health insurance.
- Confirmation from the employer that you were registered with ZUS, with the date.
- A recent payslip or settlement showing the health contribution.
- If you rely on a private policy: the full policy document, not a screenshot, showing the sum, the period, and direct payment to the provider.
- Family members: their own confirmation of being reported, or their own policy.
